Let me guess... someone cleared the vault and saved on that level. When you logged on on the same depth, it wasn't reset yet... Unlikely, since you'd need genocide to kill everything in the vault without clearing the traps or tunneling an outside wall (luring everything out implausible). I wouldn't...

having a time pass in a stable way actually made me better at roguelikes in general since I know how long resting and running takes, so I know to expect more monsters to spawn. I agree with your logic (and don't usually carry *heals with me). I should carry less =) However, I sometimes have multiple heals break, and other times use many more than I expect. I don't like to have to abandon a good vault after running out =)

Should be. Until recently, it didn't even "install" as such, you just downloaded, unzippped, and ran the EXE. As far as I know there's no necessary registry hooks - everything is self-contained in the install folder.

Quaffing a potion of death helps you revive all the high uniques from your list. I went for the third. Killed a drolem, got stuck in a forest, teleported out into a gigantic pack of ethereal hounds on the grasslands.

How the hell do you get +110 to hit? That sounds amazing... BTW forget about teleporting morgoth away. He would restore to full HP before coming back. And don't rely too much on permawalled vaults unless you have huge AC because the quaking hits will push you outside of the vault. It's nice but not...

Having plain heals is a good idea. Spam them as soon as you see yourself as a 6. Use *heal* when you see yourself as 5 or worse.
