I think he means a broken dagger of extra attacks (1d1) (+10, +2) (+2 attacks). The chance of generating an ego item in town is 0.5%, which is not that low (10% of an object being good, 5% of a good object being great).
Of course, if the drop comes from an urchin, it's just a joke, since those townies never drop loot...
