some screen with a nice tombstone telling u what killed u and the score, instead just the line in the shell "u have been killed by .."

Actually I agree on this one, it would ne nice to see your equip/inventory one more time and possibly a list of the monsters you killed?

Probably the monster that killed you. Maybe also the ghost.
